  • Gaps in diagnosis documentation were obstructing data visibility and future planning


These insights reframed how the organization thought about referrals, strategy, and capacity planning.

What We Did Together

Our work began with a leadership-level conversation about what wasn’t working—and why. We translated that frustration into questions the data could answer. Then, I analyzed one year of referral records to uncover patterns hiding beneath the surface.


By connecting referral source trends to patient outcomes like length of stay and care setting usage, we gave the leadership team a new lens on what was truly driving performance. Intake protocols were re-examined through this lens, surfacing immediate opportunities to identify higher-quality referrals earlier in the process.


To sustain momentum, we outlined a visual dashboard structure for use in quarterly leadership and board reports, helping the team move from reaction to foresight. Finally, we built an education strategy targeting high-volume referral partners—ensuring future conversations were based on real outcomes, not assumptions.


This wasn't about chasing numbers, it was about giving leadership the tools and insight to act with confidence and clarity.
